Application Requirements
  1. Personal Data – Please bring a photo identification card and social security card.
     


  2. Address History – Most recent 2 years.



  3. Assets – Need name, address, account numbers, balance and copy of most recent statements on all bank accounts (3 months).


  4. Debts – Need name, address, account numbers, balances and amount of monthly payments on all auto loans, notes to bank and active credit cards.


  5. Other Obligations – Child support/need copy of divorce degree/child care.


  6. Employment History – Past two years of employment history needed with complete names and addresses of employers. Need most recent paycheck stub and past two years’ W-2 forms. Self-employment and Straight Commission – need past two years signed, complete federal income tax returns including all schedules and year-to-date profit–and-loss.


  7. Other Income – Child Support – Copy of court history or cancelled checks for past 12 months. Rent – copy of lease agreement.


  8. Legal Documents – Bankruptcy – Need copy of discharge and schedule of debts and assets. Judgments – need released of judgment.


  9. Veterans Only – Certificate of Eligibility, DD214 (discharge papers), Original pay-stub.
